What should the final report include?
When the project is completed and you have paid all costs, it is time to submit a final report in order to receive the leader check. The final report is submitted via a digital form that includes the following parts:
- Cost summary including the following attachments if applicable:
- Invoices and receipts
- Proof of payment
- Mileage logs
- Participant lists, invitations, and other documentation for snacks, lunch, and dinner
- Information and communication materials that you have developed during the implementation
- Permits and agreements
- Other supporting documents
- Volunteer time (if you have had volunteer working hours in the project)
- Final report

How do I finalize the report?
The application is made via the form below.
Read your decision, agreement, and the handbook for those who have received a leadercheck before you finalize the report. Pay all your expenses that you need to include in the current final report. When you finalize the report, you should only report expenses that are related to the effort and that follow your support agreement.
Be sure to fill in all the information requested in the form.
Your final report must be submitted to us no later than the project's end date.
